According to a 2017 study published in the International Journal of Reproduction, Contraception, Obstetrics, and Gynaecology, the prevalence of uterine fibroids is estimated to be around 24.1% in India, with more cases occurring among women aged 30-49 years. These non-cancerous growths form in the uterine muscle wall and range in size from microscopic, pea-sized nodules to huge, bulky masses that can deform the uterine morphology. Fibroids can appear as a solitary nodule or in clusters on the inside or outside of the uterus.

Causes of Uterine Fibroids

Fibroids are unknown in their specific cause, but they are thought to emerge from an aberrant proliferation of smooth muscle cells in the uterus. Risk factors for developing fibroids generally include age, family history, obesity, and a diet high in red meat and low in green vegetables.

Types of Uterine Fibroids

Fibroids are categorized into four categories based on their location in the uterus. 

  • Intramural fibroids: They are the most frequent form and grow within the uterine muscle wall. 
  • Submucosal fibroids: They form just beneath the uterine lining and can extend into the uterine cavity. 
  • Subserosal fibroids: They tend to form on the uterine wall and can impinge on neighbouring organs. A stalk or stem-like structure connects pedunculated fibroids to the uterus.

Symptoms of Uterine Fibroids

Many women with fibroids have no symptoms, but when they develop, they can be severe and have an impact on their quality of life. Heavy or prolonged menstrual cycles, pelvic pain or pressure, frequent urination, constipation, and backache are all common symptoms. Fibroids can induce infertility in some situations or even miscarriage.

Diagnosis of Uterine Fibroids

A normal pelvic exam can reveal fibroids, but an ultrasound or MRI may be required to confirm the diagnosis and assess the size and location of the fibroids. To check the uterine cavity, a hysteroscopy, which includes inserting a tiny camera into the vagina and cervix, may be required in some circumstances.

Treatment of Uterine Fibroid 

Fibroid treatment is determined by the intensity of the symptoms, the size and location of the fibroids, and whether or not the lady desires to have children in the future. The treatment options are listed below:

  • Women with small or asymptomatic fibroids may benefit from monitoring them over time and only treating them if symptoms emerge or worsen.
  • Medications: Birth control pills, progestin-releasing intrauterine devices (IUDs), and gonadotropin-releasing hormone (GnRH) agonists can all help decrease fibroids and relieve discomfort. These drugs, however, are not a permanent treatment and may cause negative effects.
  • Surgery may be required if fibroids are causing serious symptoms or interfering with fertility. There are a number of surgical options available, including:
  1. a) Myomectomy: A surgical technique that removes fibroids while leaving the uterus intact, making it an option for women who want to have children in the future.
  2. b) Hysterectomy: A surgical operation to remove the uterus that provides a permanent solution to fibroids but is not an option for women who want to have children in the future.
  • Uterine fibroid embolization (UFE): It is a procedure used to treat uterine fibroids. It is a minimally invasive treatment that involves injecting tiny particles into the blood arteries that supply blood to the fibroids, causing them to shrink and die.
