Endoscopy/Laparoscopy

Both Laparoscopy & Endoscopy suggest Minimal Invasive diagnostic procedures.
These techniques are used for gynaecological surgeries as preferred alternatives to open surgery because they do not produce intense pain after surgery; only mild pain and discomfort is felt in contrast to invasive or open surgical procedures; leave very small scars compared to open surgeries. Shorter hospital stay, and faster healing are some other advantages of minimal invasive surgeries.
There is a fine difference between laparoscopic surgery & endoscopic surgery.

Laparoscopic surgery:
It uses a laparoscope to look inside the pelvic area. A laparoscope is a slender, lighted telescope that allows the doctor to see inside the body. Diagnostic laparoscopy helps determine conditions such as endometriosis or fibroids. It can also be used to perform a variety of surgeries. Laparoscopy involves making a small incision along the abdomen through which a small telescope can be inserted through the incision to get a good view inside the abdominal area.

Endoscopic surgery:
Endoscopy is a nonsurgical procedure that uses an endoscope - a flexible tube with a light and camera attached to it. During endoscopic surgeries, the endoscope is inserted in the body through small incisions or natural body openings.

Fibroids are growths that occur in the uterus, most commonly during childbearing years. Fibroid may be a single fibroid or multiple fibroids; extremely small or big in size, but are mostly benign or non-cancerous in nature. Sometimes fibroids may be symptom-free or may cause terrible suffering depending on the size and the number of the fibroids and their location.
The most Common Symptoms of Fibroids are:

  • Heavy menstrual bleeding
  • Prolonged menses
  • Pelvic pain
  • Frequent urination
  • Constipation
  • Backache or leg pains
Fibroids may cause extreme pain, repeated miscarriages, severe anaemia. Surgical treatment options include:

Hysterectomy

Hysterectomy means removal of the uterus (commonly called as womb). However other organs such as ovaries, fallopian tubes and the cervix are very frequently removed as part of the surgery.
The most common reasons for a hysterectomy are:

  • Fibroids - non-cancerous growths in uterus which may cause heavy or painful periods, frequent urination, constipation, and pain during sex.
  • Adenomyosis - uterus lining grows into its muscular wall, making your periods painful.
  • Endometriosis - uterus lining grows on the outside of other pelvic organs, which can cause severe period pain, ongoing back aches, uncomfortable sex, and bleeding between periods.
  • Pelvic inflammatory disease (PID) - Painful condition where bacterial infection permanently damages uterus and fallopian tubes
  • Scar tissue - that forms after a C-section or other surgery can cause pain and bleeding.
  • A prolapsed uterus - the tissues that support uterus are weakened (often after childbirth) and drop-down uterus into vagina. This causes back aches, loss of bladder control, and painful sex.
  • Bleeding in vagina – bleeding that is not part of menstrual period
  • Cancer of the cervix, ovaries, or uterus.

Most women with endometriosis have no symptoms. However, when women do experience signs and symptoms of endometriosis they may include:

  • Pelvic pain that may worsen during menstruation
  • Painful intercourse
  • Painful bowel movements or urination
  • Infertility
Treatment of endometriosis includes medication and surgery for both pain relief and treatment of infertility if pregnancy is desired.

The common causes are:

  • Pregnancy
  • Difficult labor and delivery or trauma during childbirth
  • Delivery of a large baby
  • Being overweight or obese
  • Lower estrogen level after menopause
  • Chronic constipation or straining with bowel movements
  • Chronic cough or bronchitis
  • Repeated heavy lifting
Signs and symptoms of moderate to severe uterine prolapse include:

  • A feeling of fullness or pressure in pelvis
  • Low back pain
  • Feeling that something is coming out of your vagina
  • Painful sexual intercourse
  • Difficulty with urination or moving bowels
  • Discomfort walking

Urinary Incontinence

Urinary incontinence is the involuntary leakage of urine due to loss of bladder control.
Types of incontinence are:

  • Stress incontinence: leakage of urine during activities like running, coughing, sneezing, laughing, exercising
  • Urgency incontinence: a strong, sudden need, or urgency to urinate before losing urine
  • Mixed incontinence: Mix of both these symptoms

Most common causes of incontinence in women are:

  • Childbirth - the childbirth process can damage the muscles and nerves that control urination
  • Chronic coughing - long-lasting coughing increases pressure on the bladder and pelvic floor muscles
  • Menopause - reduces production of the hormone that keeps the lining of the bladder and urethra healthy
  • Neurological problems - women with diseases or conditions that affect the brain and spine may have trouble controlling urination
  • Physical inactivity - decreased activity can increase a woman’s weight and contribute to muscle weakness
  • Obesity - extra weight can put pressure on the bladder, causing a need to urinate before the bladder is full
  • Older age - bladder muscles can weaken over time, leading to a decrease in the bladder’s capacity to store urine
  • Pelvic organ prolapse - causes sagging of the bladder, bowel, or uterus out of their normal positions
  • Pregnancy - the fetus can put pressure on the bladder during pregnancy
